In-Office Appointments
Dr. Jacobson offers both in-person office appointments and virtual/telehealth appointments via Zoom. Established patients may also schedule phone appointments.
Office visits are by appointment only. Please follow map directions to the office at 3801 PGA Blvd. Parking is free and convenient in spaces marked “visitor” or in un-marked spaces. Take the elevator to the 6th floor and enter the reception area for suite 600. Dr. Jacobson will meet you in the waiting area at your appointment time.
Scheduled in-person appointments can be changed to phone appointments or virtual appointments at any time, even if just before the scheduled start time. Please note, insurance companies may not offer any reimbursement for phone appointments and will not reimburse for missed appointments. There is a 24 hour cancellation policy.
