There are no membership or annual fees. Dr. Jacobson is an “out-of network” psychiatrist and does not participate with insurance plans.  He can provide a receipt at the time of payment with billing information and codes that can be used to seek reimbursement directly from insurance companies. 

Dr. Jacobson has opted-out of Medicare.  Medicare patients will be required to sign a private contract certifying that they will not attempt to seek reimbursement from Medicare for services provided by Dr. Jacobson.  Medicare patients may submit to their secondary insurance.

Appointments cancelled without 24 hours notice by voicemail or text will be billed at the scheduled rate if no other patient fills the reserved time.  The reserved time for in-office or telehealth appointments can always be changed to a phone appointment. All patients must have an appointment at least every two months.

Rates

Initial Evaluation Typically 60-90 minutes (350.00-450.00)

Appointments 15-20 minutes (190.00)

Appointments 25-30 minutes (250.00)

Longer Appointments are available on prorated basis

Phone calls under 5 minutes are not billed

Payment is due at the time of appointment via cash, check or credit card.
